The Plan Administrator for CHIP, Blue Cross and Blue Shield of Illinois, has the largest participating hospital provider
network in the state and over 31,000 participating primary care physicians and specialists. This network has been adopted
for use by CHIP. Refer below for a listing by region of participating hospitals in this network. When traveling out-of-state,
participants in CHIP Plans will also be able to use the Administrator’s BlueCard PPO program to receive in-network
benefits from certain hospitals in these other states. Call 1-800-810-2583 to verify which physicians and out-of-state
hospitals are currently participating in the BlueCard PPO Program.
